Clinic Policies

Colebrook Family Practice was designed with quality patient care being the focus. Therefore, there are certain clinic policies in place to help maintain our efficiency and provide a more uniform experience for all our patients.

Cancellation Policy

We require 24 hours’ notice to cancel all appointments. Each time a patient misses an appointment without providing notice, another patient is prevented from receiving care. Therefore, Colebrook Family Practice reserves the right to charge a fee of $30 for missed regular appointments and $50 for missed longer appointments, such as: complete physicals, pap smears, excisions, baby immunizations, etc. without a compelling reason for not providing 24 hour advance notice. No show fees will be directly billed to the patient and must be paid prior to your next appointment.

Lateness Policy

If you arrive more than 15 minutes after your scheduled appointment, the clinic reserves the right to reschedule your appointment and charge a missed appointment fee.

Pet Policy

Our office does not allow any pets or animals, except for authorized service animals such as guide dogs.

Test Results Policy

Our staff cannot give the results of any test over the phone, at any time. There are two main reasons for this: (1) we must ensure the confidentiality of your medical information; and (2) in certain cases, it is important for the doctor to reassess your condition in order to ensure that both they and you have a full understanding of your problem. Only the doctor can properly answer your questions and ensure that you are aware of the relevance and significance of the results.
